Position Summary:

Long Beach Alliance for Children with Asthma (LBACA) is an outpatient department of Miller Children�s & Women�s Hospital. Our mission is to change the profile of childhood asthma in the most affected areas of Long Beach and surrounding communities through improved healthcare delivery and quality, outreach, education support systems, healthy living environments, and changes in environmental health policy at all levels. We envision these communities will be a place where residents, families, children and youth, community organizations, decision makers, health care providers, schools, businesses, and community leaders are partners to improve the quality of life for children with or at risk of developing asthma. The LBACA program reaches culturally and linguistically diverse communities who experience higher rates of asthma in our region � mostly Latinos, Asian & Pacific Islander, and African American. The current Community Health Worker position will require a deep cultural understanding and network of contacts to build upon and further connect our program with African American children and their families.

Tasks Performed by the Community Health Worker:

  • Conducts home visits and visits each family as required to meet program goals.  Educates and advocates for parents & their children.  Conducts the CHW In-Home Asthma Education Program as directed by program leaders to meet the goals of the LBACA program.
  • Provides asthma education through classes at various community sites, including schools, community centers, parks, faith based institutions, hospitals, and clinics.
  • Assesses the home environment for asthma triggers and family needs that may prevent asthma management, including situations that may lead to child abuse/neglect, developmental delay, or social isolation. Communicates concerns and issues to the program manager timely and appropriately.
  • Serves as a resource for schools and community organizations.  Provides outreach as directed by program leaders to meet the goals of the LBACA program. Participates in community health fairs and special events. 
  • Maintains accurate patient files and program documentation. Follows-up with clients: keeps ongoing contact with families as required to meet program goals. Attends scheduled department meetings and gives accurate monthly progress/tracking reports. Maintains daily schedule and keeps calendar up-to-date. Completes reimbursement forms accurately and timely.  Communicates with program manager about home visits and outreach on a regular basis established between CHW and program manager.
  • Attends ongoing educational training, scheduled group meetings and other activities, including several community-based organization meetings.  Provides peer support. 
  • Assists with maintaining a library of community resources relevant to social, housing, mental health, and healthcare services.
  • Demonstrates understanding of age specific competencies for the diverse populations outlined in LBACA�s program.
  • Attends department specific staff meetings, in-services, education, and training. Participates in selected department specific process improvement projects. Participates in the orientation and development of volunteers, students and/or new staff as assigned.
  • Performs other duties as assigned.


  • Must have experience working with and be sensitive to the target community
  • Prior experience working in health focused community based programs preferred
  • Minimum education: High School Diploma
  • Knowledge of community resources is preferred
  • Willing to travel to sites throughout Long Beach and surrounding communities and greater LA area
  • Basic computer skills
  • Valid CA driver�s license and auto insurance documentation
