Tasks Performed by the Community Health Worker:

  • Conducts home visits and visits each family as required to meet program goals.  Educates and advocates for parents of children.   Conducts the CHW In Home Asthma Education Program for approximately 90 families/year or as directed by program leaders to meet the goals of the grant.
  • Assesses the home environment for asthma triggers and family needs that may prevent asthma management, including situations that may lead to child abuse/neglect, developmental delay, or social isolation. Communicates concerns and issues timely and appropriately.
  • Serves as a resource for schools and community organizations.  Provides outreach for a minimum of 8 hours a week/32 hour/ month or 384 hours per year or as directed by program leaders to meet the goals outlined in the grant.
  • Maintains accurate patient files and program documentation. Follows-up with clients: keeps ongoing contact with families as required to meet program goals.  Attends scheduled department meetings and gives accurate monthly progress/tracking reports.  Maintains daily schedule and keeps calendar up-to-date. Completes reimbursement forms accurately and timely.  Communicates with program manager about home visits and outreach on a regular basis established between CHW and program manager.
  • Attends ongoing educational training, scheduled group meetings and other activities, including several community-based organization meetings. Participates in community health fairs and special events.  Provides peer support. 
  • Provides interpreter services in the field to assist with coordination of care, as needed.
  • Assist with maintaining a library of community resources relative to social and healthcare services.
  • Demonstrates understanding of age specific competencies for the population outlined in the grant.
  • Attends department specific staff meetings, in-services, education, and training. Participates in selected department specific process improvement projects. Participates in the orientation and development of volunteers, students and/or new staff as assigned.
  • Performs other duties as assigned.


  • Must be culturally and linguistically competent and sensitive to the target community
  • Prior experience working in health focused community based programs preferred
  • Minimal education: High School Diploma
  • Knowledge of community resources is preferred
  • Willing to travel to sites throughout Long Beach and surrounding communities and greater LA area
  • Basic computer skills
  • Valid CA drivers license and  auto insurance documentation
