Position Summary

Under the supervision of a pharmacist, the Medication History Pharmacy Technician will conduct medical history interviews with patients and /or patient�s family, identify patient�s own medication, and review and verify patient�s medication use from reliable source(s).  The Medication History Pharmacy Technician will transcribe patient�s medication list and document medications, including dose, route, and frequency, into the electronic health record (EHR).  The Medication History Pharmacy Technician must maintain skills necessary for an inpatient pharmacy technician and maintain an overall awareness of inpatient pharmacy operations.

Essential Functions and Responsibilities of the Job

  • Effectively meets customer needs; build productive customer relationship
  • Accurately collect and enter patient�s prior to admission medication list into the hospital�s EHR
  • Anticipates and generates creative approaches to addressing problems and opportunities
  • Allocates time and resources efficiently and effectively; prioritizes work and delegates as appropriate.

Minimum Requirements

Qualifications/Work Experience:

  • One (1) years of hospital pharmacy inpatient technician experience and successful completion of a recognized pharmacy technician training program.
  • Experience with medication history/reconciliation preferred.
  • Knowledge of medications including different formulations, brand/generic, and common indications.


  • Must possess a valid Pharmacy Technician certificate issued by the California State Board of Pharmacy.
