Purpose Statement / Position Summary:

This position will be responsible for the care of patients requiring an attending physician while on hospice service. The NP care will be coordinated with the Hospice Medical Director. The NP is defined as a Registered Nurse who is permitted to perform such services as legally authorized to perform under the review and signature of the Hospice Medical Director in accordance with California state law and regulatory mechanisms. Medical services provided must be reasonable and necessary, be included in the plan of care and must be  services that, in the absence of the Nurse Practitioner would be performed by a physician.
This position requires a full understanding and active participation in fulfilling the mission of Saddleback Memorial Medical Center and Hospice of Saddleback Valley. It is expected that the employee demonstrate behavior consistent with core values. The employee shall support Saddleback Memorial Medical Center�s strategic plan and the goals and direction of the performance improvement plan.

Minimum Requirements / Work Experience:
� Four years clinical nursing experience with 2 years in hospice care
Education / Licensure / Certification:
� Current RN License and Nurse Practitioner Certification in the
State of California
� Masters Degree in nursing
� Current BCLS-C
� Graduate of an accredited nurse practitioner program
� Current California Driver�s license
� Demonstrate evidence of current automobile insurance as required
by California law
