$3,500 signing bonus available to external hires new to MHS employment, contingent upon satisfaction of employment requirements.

Position Summary

The OB Intake and Navigation LVN plays a critical role in the early stages of prenatal care by conducting comprehensive virtual visits for new obstetric patients. This position focuses on gathering detailed medical and genetic histories, educating patients on prenatal care, and initiating appropriate testing and follow-up steps in collaboration with the care team. The LVN will incorporate the five (5) Simply Better steps with every patient and team member interaction.

Essential Job Outcomes

·Meet ambulatory nursing clinical competencies for LVN licensure.

·Demonstrate understanding and adhere to the Scope of Practice based on role/licensure/education.

·Patient Intake & History Collection

oConduct full medical, surgical, obstetric, and genetic history intake for all new OB patients.

oDocument findings directly into the electronic medical record (EMR) using appropriate flowsheets and tools.

oIdentify and flag high-risk conditions for provider review.

·Prenatal Genetic Counseling Support

oReview and explain available prenatal genetic testing options (e.g., cfDNA, carrier screening).

oAssist patients in selecting appropriate tests based on clinical guidelines and patient preferences.

oInitiate orders for labs and ultrasounds as appropriate, per standing protocols or provider direction.

·Patient Education

oProvide counseling on prenatal vitamins, nutrition, exercise, and lifestyle modifications.

oEducate patients on substances to avoid during pregnancy and general pregnancy precautions.

oShare resources such as pregnancy guides, appointment schedules, and community support materials.

·Identify improvement opportunities, implement countermeasures and escalate to appropriate management resource as needed.

·Maintain up-to-date knowledge. Capability to perform all assigned nursing functions and competencies to float to all clinical roles within scope of licensure.

·Collaborate with all departments to coordinate patient care.

·Ensure documentation is accurate, complete and timely; reflecting patients’ plan of care.

·Provide clear direction to all members of the care team to support the plan of care while maintaining the responsibility and accountability for the delivery of ambulatory nursing care.

·Demonstrate continued development of clinical expertise, current knowledge of healthcare trends and education related to ambulatory nursing practice.

·Participate in processes that improve Standards of Care and practice in the delivery of ambulatory nursing care.

·Implement a teaching plan with patient, family and/or caregiver(s) as necessary.

·Perform any additional or miscellaneous duties as requested by the management team within the scope of knowledge and ability.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Experience

·         Licensure: Must hold a valid California Licensed Vocational Nurse (LVN) license. 

·         Clinical Knowledge: Minimum 1 year experience in OBGYN or women’s health; Strong knowledge of OB/maternity care is required;

·         Experience in case management, managed care, or a related healthcare coordination role is preferred. 

·         Bilingual (English/Spanish) preferred

 

Education

• Current California LVN License with IV Certification required

• Current BLS for Healthcare Providers required
