Position Summary

Under minimal supervision and in a professional and friendly manner, this position has responsibility for the day to day clinical operations of Case Management, Specialty Programs within the Medical Management department.These programs include, but are not limited to Behavioral Health, Palliative Care, Advanced Care Planning and Hospice amongst others that staff reporting to this manager are supporting.Populations served within these specialty programs include Memorial Care Medical Foundation’s HMO, ACO, Medicare FFS as specialty programs are developed.

Essential Functions and Responsibilities of the Job

  1. Effectively manage the day to day operations of assigned area by applying advanced critical thinking, effective, professional verbal and written communication as well as employee engagement strategies for Case Management Specialty Programs including but not limited to:
  2. Behavioral Health
  3. Palliative Care, Advanced Care Planning, Hospice
  4. Social Worker staff
  1. Develop written and approved workflows, program descriptions and policies for the programs responsible for and within a timely manner.
  2. Participate in all program related calls and committees internally and externally on a regular basis and communicate to Director via written and verbal outcomes, action items, barriers and metrics in a timely manner.
  3. Ongoing offsite visits to programs and staff to ensure consistent communication and collaboration.Will also require in office visibility for meetings and leadership interaction.
  4. Along with the department Director active involvement with department budgetary process within each fiscal year including planning and monthly oversight to potential and real variances. Timely communication to reporting manager and corrective action to ensure future budget alignment.
  5. Support overall employee engagement strategies within teams and via Supervisors that align with the organizations positive impact on overall outcomes and achieve performance quartile as set forth via department and/or organization.
  6. Selection, orientation, training, coaching, attendance, payroll and performance monitoring and coaching of staff managing.
  7. Develop, maintain and revise Lean Management Tools including; Visual Management Systems, Huddles and PDSA activities with Supervisors and Staff on a regular basis and that align to unit and department strategic goals and initiatives.
  8. Produce monthly, or as assigned, data information to Medical Management Leadership, including the following:
  9. Create, review and submit per schedule, program aligned visibility boards (Executive and Department level) and other defined reports aligned to the programs managed.
  10. Collaborate and communicate timely to Director questions and concerns to management regarding data reporting related to programs managing
  11. Manage staffing within each program to ensure the following:

I.Behavioral Health (BH Programs)-

Collaborative Care-

oCaseloads for BH programs and staff should be based on and tracked based on defined metrics for each of the BH Specialists.

oIdentify and report to manager monthly or as defined as well as when caseloads are impacted.

oAppropriate billing that align to program

II.Silver Cloud-

oManage BH Specialist and/or coach workload and ensure reviews are completed on time.

oReport on number of clinics, staff, and providers educated on Silver Cloud

oEducate on and promote Silver Cloud with MCMG and GNP clinics, include ensuring they have patient education materials

oMeasure users on a weekly or as-needed basis using Silver Cloud reporting:

oActive Users (total), Current Users, and Completed Users

oUsers per program

oReport on significant change metrics for Anxiety and Depression users

III.Telehealth tools and applications –

oPromote and train existing staff within Behavioral Health to utilization and spread Telehealth capabilities to the programs managed.

IV.Palliative Care, Advanced Care Planning and Hospice-

oOversite to all aligned, preferred and contracted Palliative and Hospice Programs.

oMemorial Care Home Based Palliative Care Program-

·Measure monthly referrals and enrollment to Home Based Program for all Business Segments and report to Leadership

oPartner and collaborate with Palliative Care Program to ensure consistent and quality workflows, reporting and outcomes

oMeasure cost of care outcomes for patients enrolled within a Palliative Care program.

oWork with Palliative Care Medical Director and Medical Management Leadership ongoing to promote growth to Palliative Care Services.

oReport metrics and outcomes on regular basis in department and leadership.

oCollaborate as needed across all Medical Management teams should cross coverage and workflows exist.

V.Social Worker Services-

oManage day to day operations of staff supporting all Social Work Services within and outside of specialty programs.

oIncludes- Productivity measurement standards for activities supporting (General Social Work support to MCMG, GNP), ACTIVE Diabetes, and Palliative Care Programs.

oCreate, review and complete all required reporting to activities supported.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Qualifications/Work Experience:

  1. Minimum three years of previous Manager role in Health Care related position
  2. Strong Social Work, Behavioral Health and Population Health background
  3. Exceptional verbal, written and critical thinking skills
  4. Proficiency in Microsoft Word suite, Electronic UM/CM Medical Records platform (Epic, Next Gen, Sunrise or like) and other Analytic tools  (ex- Health Catalyst)

Education/Licensure/Certification:

  1. Bachelor of Science in aligned field or higher
  2. MSW or LCSW
