Position Summary

Pharmacy technicians play an important role in Medication Therapy Management (MTM) through patient chart reviews, outreach, and supporting the overall program coordination. It requires basic knowledge of medications, conditions and knowledge of MTM services. Additional responsibilities include communications with pharmacists and physicians as appropriate.

Essential Functions and Responsibilities of the Job

• Conduct patient outreach under the guidance and supervision of the pharmacists for Part D measures
• Maintain a thorough understanding of pharmacy measures and review health plan data to determine performance gaps as appropriate and determine outreach focus
• Partner with MTM team to identify and address overall performance gaps
• Submit data as appropriate to health plans and enter data into Pharmacy applications and vendor platforms
• Collect and enter supplemental data for MTM
• Communicate clearly and thoroughly, provide excellent customer service and be able to work both independently and as part of a team.
• Supports clinical and utilization review duties related to specific pharmacy-related quality metrics
• Conducts patient outreach to address pharmacy related care gaps and escalates patient questions or concerns to the pharmacist as appropriate
• Assist/obtain resources needed to support provision of clinical pharmacy services.
• Work collaboratively within the Performance Improvement department and health plans in joint strategic initiatives.
• Support provider and patient communication on relevant clinical and quality updates.
• Contribute to team or departmental projects directed toward the achievement of improved service efficiency and patient outcomes
• Support implementation of quality initiatives for clinical pharmacy services including but not limited to outreach and documentation.
• Other related duties and projects, as assigned.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Qualifications - External

Experience

·         A minimum of 1 year of medical office and pharmacy technician experience.

·         Experience with medication therapy management (MTM) preferred

·         Basic PC skills including Microsoft Word, PowerPoint and Excel required

Education

• High school graduate or equivalent required.
• Certificate from an accredited Pharmacy technician program required.
• Bilingual preferred
