Pay Range*: $56.00/hr - $81.00/hr

Position Summary

The position is responsible for providing patient specific orientation and education to both new and old staff including Nursing Staff who are being re-skilled for purposes of functioning as Care Coordinators throughout Memorial Care health System. This position assists with developing a comprehensive plan for meeting the educational needs of the Care Coordination staff and carries out that plan to support high quality education for Care Coordination

Essential Functions and Responsibilities of the Job

  • Orientation Coordination and Planning
    • Ensures orientation of new cm staff on all topics as outlines in orientation policies
    • Ensures re-orientation of cm staff as appropriate
    • Develops individual orientation plan for new cm staff
    • Works with the System Ex Dir Care Coordination to develop preceptor standard work for Memorial Care
    • Ensures Orientation and educational records of CM staff are completed
  • Staff Development
    • Performs annual needs assessment
    • Function as Interim onsite preceptor as needed
    • Create/maintain Student program with surrounding Colleges in OC and LA County areas
    • Provide Regulatory updates on CMS/HINN letters/2 midnight rule/Ortho In-patient only list/Pepper Data/Title 22/AB1203/Code44 and 72
  • Able to function at the Manager CM Level at any Memorial Care Location on Interim basis as needed and as determined by Executive System Dir Care Coordination
  • Able to articulate reasons for Care Coordination activities to new CM staff
  • Able to increase overall knowledge of Care Coordination Staff
  • Works with Social Work Leadership at Memorial Care Hospitals to develop educational modules for SW teams
  • Other duties as assigned

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

Qualifications/Work Experience:

  • 3 years/min Acute Hospital Based Care Coordination Services and or 3 years/min Managed Care/Ambulatory Space Care Coordination Services

  • At least 1 year of Hospital leadership experience in any department

Education/Licensure/Certification:

  • Required BSN. Preferred Master's Degree  
  • Required active RN California license
