Position Summary

LVN Visiting nurse to perform visits of Hospice and or Palliative patients in their home to perform assessment of patient's condition, patient teachings and perform procedures like wound care, Foley Cath and other nursing procedures and visit types on PM/ and night shit weekends.  Follows plan of care under the direction of the RN. 

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Experience

·       One year experience in hospice is preferred

·       Excellent customer service

·       Computer skills a must

Education

·         Licensed Vocational Nurse Certification required

·         Current Basic Cardiac Life Support (BLS) Card
