Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities.

The Cardiac Nurse Navigator collaborates with other professional disciplines/unit leaders to ensure safe and effective patient care delivery and the achievement of desired patient outcomes for the cardiac population. They are responsible for facilitation of nursing practice and continuum of care outcomes, in collaboration practitioners and physicians. The Cardiac Nurse Navigator coordinates patients' care from pre-admission, hospitalization, to post-discharge follow-up in collaboration with the healthcare team, to best meet the patients' physical, emotional, and spiritual needs. The Nurse Navigator uses the nursing process that includes assessment, problem identification, goal definition, plan development, evaluation, reassessment and plan revision, and documentation in the patient record to achieve optimum patient outcomes on both an individual and population basis.

Essential Functions and Responsibilities of the Job

  1. Demonstrates the ability to safely and competently perform all aspects assessment, planning, implementation and evaluation of patient care in the clinic.
  2. Contributes to an effective and safe working environment.

  4. Demonstrates knowledge and understanding of patient privacy rights.
  5. Maintains confidentiality of all medical, financial and other sensitive material in printed, electronic or verbal form, which may jeopardize the privacy of patients and or employees.
  6. Demonstrates the ability to use good customer service skills by managing and acknowledging patient-family-significant other expectations.
  7. Demonstrates tact, sensitivity and discretion.
  8. Demonstrates the ability to integrate knowledge of job specific competencies.
  9. Demonstrates continual development of clinical expertise and current knowledge of healthcare trends and education related to performance.
  10. Ability to be at work and be on time.
  11. Ability to follow company policies, procedures and directives.
  12. Ability to interact in a positive and constructive manner.
  13. Ability to prioritize and multitask.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

Qualifications/Work Experience:

  • Minimum of (5) five years’ experience in nursing and (3) years of experience in area of specialty required
  • Excellent interpersonal and communication skills
  • Experience with Electronic Medical Records preferred

Education/Licensure/Certification:

  • Current RN license to practice in the State of California required
  • Bachelor’s degree in Nursing required
  • Current American Heart Association Basic Cardiac Life Support Card required
