Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The Care Coordination Supervisor oversees daily operations for Care Coordination and Discharge Planning, ensuring compliance with all regulatory and payer contract requirements. The individual reports directly to the Manager of Care Coordination, who provides direct oversight and direction for both operational and clinical service needs. The individual can coordinate care management functions not only for individual patients, but also to lead care management for multiple patients. Responsibilities include performing audits and projects for the department, in addition to a care management caseload. As a front-line leader, this individual is also accountable to assist in the evaluation, development, and retention of department staff (licensed and unlicensed).

The Care Coordination Supervisor collaborates with other multidisciplinary disciplines to ensure safe and effective patient care delivery and the achievement of desired outcomes. This individual is responsible for the facilitation of appropriate length of stay and continuum of care outcomes, through the care management of all systems and services required for an organized, multidisciplinary team approach to care. The Care Coordination Supervisor uses a problem-solving process that includes assessment, problem identification, goal definition, plan development, monitoring, evaluation, reassessment, and revision of the plan, and documentation in the patient record to achieve optimum patient outcomes on both an individual and population basis.

Essential Functions and Responsibilities of the Job

1. Leads and/or participates in the planning, directing and the implementation of assigned projects.

2. Encourages patient-focused care through multidisciplinary rounding and ELOS (Outlier) rounds as assigned by the Manager of Care Coordination.

3. Assists in the development, monitoring and sustainment of budget requirements.

4. Effectively coordinates and oversees payroll, manages patient flow, and ensures optimal staff scheduling on a monthly basis, along with daily assignments and handling of sick calls.

5. Participates in recruitment and retention of care managers and discharge planners.

6. Under the leadership of the Manager of Care Coordination, confidently conducts audits to ensure full compliance with regulatory standards.

7. Takes on rotational on-call duties and provides care manager relief as needed, including on weekends or as directed by the Manager of Care Coordination.

8. Responds to and resolves difficult and sensitive patient care issues and customer inquiries and/or complaints.

9. Participates in activities to maintain magnet status.

10. Responsible for taking on up to a full patient load assignment for department coverage as needed.

11. Ability to be at work and be on time.

12. Ability to follow company policies, procedures and directives.

13. Ability to interact in a positive and constructive manner.

14. Ability to prioritize and multitask.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Qualifications/Work Experience:

  • 1-2 years of supervisorial experience preferred.
  • Demonstrated supervisorial experience including budget, productivity enhancement, coaching, counseling and evaluations preferred.
  • A management style that is open, team oriented, collaborative and based on trust and integrity. Effective written and verbal communication skills preferred.
  • Worked 1 year minimum in Acute Hospital Base CM Department and experience working alongside physicians, administration, and hospital staff preferred.

Education/Licensure/Certification:

  • California Registered Nurse license required.
  • Bachelor’s degree in nursing required.
  • American Heart Association Basic Cardiac Life Support (BCLS) certification required.
  • Case Management Certification preferred.
