Position Summary:
The Clinical Pharmacy Resident position is primarily designed for a recent pharmacy school graduate who wishes to spend a post-graduate year gaining further training in acute care hospital practice. In addition to gaining focused acute care experience to enable them to independently staff in all areas of the department, the Clinical Pharmacy Resident will receive training in management, complete several projects including a research project, and participate in the pharmacy student teaching program. This residency program adheres to the residency standards set for by the American Society of Health System Pharmacists (ASHP). This residency position is offered only to residents who have matched with this program by the National Matching Service (NMS). As set forth in the ASHP residency standards, residents must be licensed as pharmacists in the state of California for two-thirds of the residency year. Residents not licensed by 120 days from the start of the residency program will be evaluated for program continuation., as outlined in the Residency Contract that was provided to the resident when they matched with the program
Essential Functions and Responsibilities of the Job
1.Reviews medication and IV therapy orders and patient profiles; is able to identify the appropriateness of prescribed therapy relative to individual patient requirements and current departmental policies and procedures; and takes appropriate action to clarify or change inappropriate or incomplete medication and IV therapy orders.
2.Demonstrates a solid working knowledge of common drugs, indications, dosages, monitoring parameters, pharmacokinetic principles, common medical conditions, clinical diagnoses, and the cost complication of common treatment regimens. The Clinical Pharmacy Resident demonstrates the ability to conduct a thorough patient medication history, to be able to enter it correctly into the electronic medical record and is able to accurately review a medication history if obtained by a pharmacy technician.
3.Demonstrates proficiency in the area of pharmacy operations, patient care and patient safety activities which measurably improves patient care outcomes, reduces, overall cost of care, and/or improves the efficiency of delivering pharmacy services.
4.Directs the daily activities of pharmacy technicians and other ancillary personnel and works collaboratively with all health care team members in an appropriate and effective manner in Dorder to optimize the productivity of the pharmacy
5.ocuments any medication errors, adverse drug reactions, and interventions on behalf of patients to meet regulatory requirements, lead to process improvements, and determine optimal use of department resources.
6.Designs and completes at least one clinical or administrative research project suitable for publication and presentation at the regional resident, or other appropriate, conference. The Clinical Pharmacy Resident creates and presents a continuing education program for the Pharmacy Department Staff. The Clinical Pharmacy Resident completes other drug evaluations, medication use evaluations, and other projects as assigned. The Clinical Pharmacy Resident will have opportunities to teach pharmacy students and other health care professionals in their service area
7.Performs other duties as assigned.
*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements:
Qualifications/Work Experience
· Outstanding organizational and communication skills.
· Ability to exercise independent judgment and prioritize a diverse workload.
· Proficient understanding of common computer programs used in pharmacy practice and ability to type at least 30 words per minute preferred
Education/Licensure/Certification
· Graduate of school of pharmacy with a Doctor of Pharmacy (Pharm.D.) degree.
· Licensure as a Pharmacist in the State of California during the first 120 days of their residency year.
· Current American Heart Association Basic Cardiac Life Support (BCLS) certificate.
