Position Summary
The Licensed Clinical Social Worker (LCSW) is responsible for providing interventions to remove psychosocial barriers preventing safe and timely transition through the continuum of care. Skillfully advocates in empowering patients to maintain a level of control over their treatment and for self-determination. The professional licensure allows this individual to provide clinical and casework supervision for staff in accordance with the needs of the department.
Essential Functions and Responsibilities of the Job
1.Independently manages a specific caseload of patients as identified by the Social Work Service Department. Evaluates and assesses to determine those social, emotional, financial and cultural factors of the patient’s life situation that may contribute to the total diagnosis and treatment plan for the patient. Demonstrates a comprehensive and systematic approach in providing a variety of therapeutic and intervention modalities to remove psychosocial barriers that prevent a safe and timely transition through the continuum of care.
2.Provides leadership and healthcare team coordination to address protective service issues, life care decisions, and advance directives. Actively manages a treatment plan to address complex psychosocial issues of high-risk patients. Provides crisis intervention services to patients and families and the health care team.
3.Collaborates with the case manager and the healthcare team to provide education, resources, and referrals to patients, families and caretaker. Ensures that psychosocial and socioeconomic needs are addressed in the treatment or discharge plan. Facilitates coordination among health care professionals, services, and settings involved in the patient’s care, with a focus on enhancing patient satisfaction through ensuring that the rights of the patient and family are respected and maintained throughout the continuum of care.
4.Maintains professionalism and provides ongoing education to the healthcare team regarding the social workers role and psychosocial issues as it relates to patient care.
6. Educates, facilitates and coordinates with the patient, families, caretakers, and community agencies to ensure that resources available to the patient are in place to prevent readmission to the hospital; ensuring effective and efficient utilization of private, public, and philanthropic funds.
7.Responds to and completes psychosocial services in a timely and efficient manner, causing no avoidable delay in the implementation of the treatment plan or in the anticipated discharge time or date. Manages complex biopsychosocial cases to expedite patient and family follow through of treatment plan.
8.Ability to be at work and on time. It is required to be on the campus of the medical center to accommodate issues that may arise out of a crisis and to support the medical team, peers and staff and their assignment to a specific area.
9.Is required to be on-call on a rotational basis as per the Department Director.
10.Works collaboratively with the multidisciplinary healthcare team and community agencies to provide education, resources and referrals.
11.Participates in consultation and coordination of services in the hospital and with community agencies as required or appropriate.
12.Attends and participates in specialty team rounds and patient care conferences as appropriate to case load assignments.
13.Plans care coordination activities for children with discharge needs that require coordination with other community agencies who will be helping monitor clinical issues or provide services.
14.Communicates with community agencies to facilitate continuity and carryover of services for client.
15.Conducts psycho-social assessments, supportive counseling and interventions with patients and families and documents in the Electronic Medical Record (EMR) at the point of service.
16. Navigates systems and advocates to assist patients’ in accessing appropriate resources and services.
17. Makes mandated protective services reports based on the psycho-social assessment of patient and family.
18. Addresses barriers to a safe and timely discharge.
19. Provide clinical and casework supervision to staff based on the needs of the department.
20. Ability to effectively plan, initiate, and facilitate education and support groups for patients and families to assist with coping and psycho-social issues related to disease process.
21. Ability to follow company policies, procedures and directives.
22. Ability to interact in a positive and constructive manner.
23. Ability to prioritize and multitask.
24.Performs other duties as assigned.
Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Experience
- Three to five years direct social work experience in an acute healthcare setting preferred. Ability to provide clinical and casework supervision to MSW colleagues preferred.
- Ability to act as a Field Supervisor and/or Preceptor to MSW Student Interns preferred.
- Competency is defined and measured via the Social Worker Job Specific Competency Assessment and Checklist. Each Social Worker, LCSW is assessed at orientation and annually using these tools.
Education/Licensure/Certification
- Masters of Social Work (MSW) Degree from an Accredited School of Social Work required.
- Licensed Clinical Social Worker from the CA Board of Behavioral Sciences (LCSW) required.
- California Children’s Services paneling required within 90 days of employment.
