Position Summary

The Welcome Baby (WB) Program is a free, voluntary home visitation program which includes visits with the family prenatally, in the hospital, and during the postpartum period. The purpose of the program is to provide primary health prevention and parent education and to invite and welcome families into an array of services and support elements developed in their community.  The Welcome Baby Outreach Specialist's responsibilities will include community outreach at health centers, provider offices, and other social service sites aimed at recruiting and enrolling eligible clients into the program. They will be responsible for developing relationships w/ key community contacts, maintaining prenatal outreach logs, following up w/ potential clients, and conducting client intakes in a sensitive and nonjudgmental manner. 

Essential Functions and Responsibilities of the Job

  1. The Welcome Baby Outreach Specialist will develop and maintain relationships and provide outreach services to enroll clients at both provider offices and community organizations.
  2. Contact potential clients from outreach efforts to encourage participation in the program and enroll them if appropriate.
  3. Identify appropriate client resources and provide referrals as needed.
  4. Retain documentation on outreached clients for future reports for Hospital and Welcome Baby.
  5. Attend and complete all required trainings for the Welcome Baby Program.
  6. Attend and participate in all staff meetings and reflective supervision opportunities.
  7. Works closely with all Welcome Baby staff to provide a continuum of services.
  8. Complete other duties as assigned

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

Qualifications/Work Experience:

  • At least one year of experience in community outreach
  • Bilingually proficient in English/Spanish
  • Proficient with all Microsoft Office programs
  • Strong writing, and organizational skills
  • Ability to interact with community members, physicians, physician office staff, and community organizations, community leaders and hospital employees

Education/Licensure/Certification:

  • Bachelor’s degree preferred in child development, social work, psychology, human development
  • Must have current CPR certification by American Heart Association.
  • Must possess and maintain a valid California State Driver's License, valid vehicle registration, and liability insurance.
