Position Summary

The Pharmacist is responsible for improving therapeutic outcomes for the patients through safe, rational and cost-effective pharmaceutical care, education, and research. The Pharmacist collaborates with members of the health care team to provide accurate and timely pharmacy services. The Pharmacist maintains competency in drug therapy and disease-state management. The Pharmacist independently manages Medical Center approved Pharmacy regulated therapies and participates in Pharmacy clinical intervention program. The Pharmacist provides drug information to Physicians, Nurses, and other health care workers to improve the quality of medication prescribing and administration. The Pharmacists directs the work of ancillary pharmacy personnel. The Pharmacist educates patients and families to improve compliance with drug therapy.

The Infusion Center Pharmacist will collaborate with Infusion Center staff, Nursing, Pharmacy, providers, and others to provide care to infusion center patients. Utilizing EPIC and EPIC Beacon, this pharmacist will manage therapy plans, review and verifications medications, and approve sterile products preparation. The Infusion Center Pharmacist will educate patients and staff on medications and related considerations, perform performance improvement, provide clinical knowledge to the build and oversight of EPIC Beacon. This pharmacist will also have responsibilities in inventory management and drug purchasing for infusion center. Coverage in various infusion centers and main pharmacy may be required. 


Essential Functions and Responsibilities of the Job
1. Responsible for Pharmacy operations in the medication use process.
2. Provides comprehensive pharmaceutical care
3. Acts as a resource for drug information and therapeutic recommendations for all providers and patients.
4. Manages Pharmacy regulated therapies based on established departmental guidelines
5. Supervises and directs the activities of Pharmacy Technicians and other ancillary personnel.
6. Documents all medication errors, adverse drug events, and clinical interventions.
7. Ability to be at work and be on time
8. Ability to follow company policies, procedures and directives
9. Ability to interact in a positive and constructive manner
10. Ability to prioritize and multitask

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Education/Licensure/Certifications:
Graduated from an accredited School of Pharmacy (required)
Current California Registered Pharmacist license (required)
Completion of an ASHP accredited PGY-1 pharmacy residency (preferred)
Current Certification from Board of Pharmacy Specialties (e.g., BCPS, BCOP, etc) (preferred)
