Location: Fountain Valley, CA (Predominately remote with occasional onsite trainings & meetings)

Department: Performance Improvement

Status: Full Time

Shift: Days (8:00 am to 5:00 pm)

Pay Range*: $50.90 to $73.80

Bonus: $5,000 sign-on

Position Summary

The Medication Therapy Management (MTM) RN is responsible for medication reconciliation. This position is responsible for identifying patients needed for medication reconciliation and for preparing lists requiring pharmacist consultation.

Essential Functions and Responsibilities of the Job

  • Review health plan and discharge data to identify patient population requiring medication reconciliation
  • Ability to effectively review medication lists and complete reconciliation function
  • Outreach to patients to review medications
  • Outreach to physicians for any discrepancy on medication lists that need to be addressed
  • Prepare medication lists for patients requiring a pharmacist consultation
  • Successfully learn and navigate various computer programs And Epic/Nextgen EMRs.
  • Report performance data as requested
  • Communicate clearly and thoroughly, provide excellent customer service and be able to work both independently and as part of a team.
  • Other related duties and projects, as assigned.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

  • A minimum of 2 years as an RN, familiarity with medication reconciliation preferred
  • Basic PC skills including Microsoft Word, PowerPoint and Excel required

 

Education

  • Current California Registered Nurse (RN) license required.
