Position Summary
This position serves as a liaison between Hospice and the Community, Case Managers, Physicians, Patients and Families to ensure continuity of quality care to the hospice patient. This position facilitates & assists in the day-to-day operations of the intake process and is accountable for a smooth admission throughout a patient's/family's transition from admission to the Interdisciplinary Care team. This position will ensure prompt and appropriate delivery of patient care in accordance with physician orders and State & Federal regulations.
This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities.
Essential Functions and Responsibilities of the Job
- Ability to perform Patient Intake & Referral processes and/or the insurance per industry standard.

Minimum Requirements
Qualifications/Work Experience:
- One-year practical experience as an LVN and knowledge of home health and or hospice required.
- Excellent computer skills.
- Time management and organizational skills, attention to detail, and a focus on completion of tasks, are critical.
Education/Licensure/Certification:
- Current California LVN license required
