Position Summary
The Pulmonary Rehabilitation Coordinator is an advanced healthcare professional who utilizes education, diagnostic, and therapeutic modalities to design individualized treatment plans to improve the quality of life for each patient. The Pulmonary Rehabilitation Coordinator is responsible for assessing, planning, implementing, and evaluating the rehabilitative needs of the patient with chronic lung disease. Provides consultative services to physicians, nurses, respiratory therapists and referral sources as requested to facilitate patient discharge, and home health/DME consultation. Initiates and implements all aspects of outpatient pulmonary rehabilitation services as prescribed by the ordering physician and department medical directors. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
Essential Functions and Responsibilities of the Job
- Solicits prescriptions and payer approval. Charges appropriately.
- Interviews client, verifies referral, and seeks approval. Performs patient evaluations.
- Organizes team conferences. Provides safe environment for patients and families.
- Monitors patients and adjusts treatment plans. Provides instruction to staff.
- Efficient patient scheduling and cost management.
- Ensures compliance with guidelines. Documents, assesses, and reports outcome information.
- Utilizes and adapts to technology, computer systems and applications.
- Effective Oral and Written Communication.
- Demonstrates effective time management.
- Ability to manage classroom topics and lead exercise physiologists.
- Ability to coordinate therapy with Physicians and other providers.
- Provides expert knowledge in pulmonary disease management.
*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements
Qualifications/Work Experience:
- Respiratory Care Practitioner with 5 years experience in outpatient setting.
- Excellent communication skills and ability to teach in group setting.
- Experience with epic and insurance verification preferred.
Education/Licensure/Certification:
- Registered Respiratory Therapist (RRT)
- Current BLS
- COPD Disease Educator
- B.S. in Health Science or Respiratory Care preferred
