Position Summary
This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities.
The RN, Clinical II, Outpatient provides intermittent visits to the terminally ill and palliative patients and family, providing symptom management, and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing, and psychosocial support to provide appropriate care as the patient transitions to the end of life.
Essential Functions and Responsibilities of the Job
·Utilizes the nursing process and practice standards as a framework for the management of patient care.
·Analyzes the assessment data to determine a nursing diagnosis and appropriate interventions.
·Demonstrates leadership in the coordination of patient care across the healthcare continuum with progress towards meeting desired patient outcomes.
·Demonstrates the ability to utilize effective critical thinking skills.

·Demonstrates knowledge of the clinical information system and applies principles to patient documentation.

Minimum Requirements
Qualifications/Work Experience:
- 1- 2 year’s relevant experience as a Registered Nurse in an acute care setting. Home Health, Oncology, and/or Hospice experience preferred
- Effective problem-solving skills; good interpersonal and communication skills; ability to relate to a variety of health professionals in different settings
- EMR experience preferred
- Candidate must be available to work at least 2 days per week and pick up a rotating monthly, afterhours, on-call.
Education/Licensure/Certification:
- Current RN license issued by the California Board of Registered Nursing
- BSN preferred
- BLS issued by the American Heart Association
- Hospice and Palliative Care Certification preferred
