Position Summary
The Formula Room Technician (FRT) is responsible for the safe preparation and delivery of specialized enteral formulas and pasteurized human donor milk (PHDM) as ordered in the electronic medical record (EMR). The FRT adheres to preparation techniques in accordance with all sanitation regulations and standards established by regulating agencies and the medical center, including all policies and procedures. Duties include (1) operating formula mixing related equipment (2) maintaining proper sanitation of the formula room (3) securing the formula room (4) completing all required logs (5) performing mathematical calculations related to formula recipes (6) proper mixing, labeling, and delivery of formula. Contact with others: The FRT will regularly interact with the Formula Room Coordinator, Director of Clinical Nutrition, Registered Dietitians, and Registered Nurses. They may also interact with pharmacists, physicians, unit secretaries, and parents.
Essential Functions and Responsibilities of the Job
·Preparartion and delivery of specialized enteral formulas and PHDM including order verification, labeling, and necessary mathematical calculations.
·Inventories and maintains sufficient formula room stock. Receives and inspects PHDM.
·Prepares and maintains logs.
·Maintains sanitation in preparation area.
·Utilizes the chain of command to communicate problems with orders, storage, or administration.
·Participates in the training of newly hired FRT.
·Participates in Performance Improvement and LEAN projects including data collection.
·Performs other duties as assigned.
*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Qualifications/Work Experience:
· Experience in Formula Room setting preferred.
· Able to understand and perform algebra.
· Able to work independently.
· Close attention to detail.
· Offers to assist others when needed.
· Good interpersonal and communication skills.
· Organizational and time management skills.
· Basic computer skills.
Education/Licensure/Certification:
· Some college or an Associates degree. Nutrition Science focus preferred.
· Must pass Formula Room mixing competency exam with 90% grade or higher at the end of the training period. Passes annual competency tests.
