Position Summary

This position provides physical therapy services to adolescent, adult and geriatric patients in order to facilitate the highest level of independence and recovery from or adaptation to various deficits or disabilities through: providing evaluation and assessment, functional activities, therapeutic exercise, procedures, education and training.

Essential Functions and Responsibilities of the Job

  • Evaluates adolescent, adult and geriatric patients. Develops individualized treatment plans and patient centered goals. Monitors and documents patient progress and modifies treatment plan as needed.
  • Provides patient treatment and patient / family education for each assigned patient in accordance with the identified treatment plan and goals.
  • Schedules and coordinates patient care in collaboration with interdisciplinary team.
  • Maintains all required documentation for each patient in compliance with regulations and expectations of assigned area.
  • Supervises therapy assistants with clinical functions. Meets with assistants to update treatment plans when acting as primary in a given program area.
  • Supervises rehabilitation technicians as they act as care extenders in the clinical setting.
  • Responsible for developing individual knowledge base of best practices, new techniques, technologies through continuing education activities.
  • Has an awareness of and participates in continuous departmental quality improvement projects as able.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Qualifications/Work Experience:

  • No previous work experience necessary.

Education/Licensure/Certification:

  • Master’s or advanced degree in Physical Therapy (or equivalent) from approved Physical Therapy program.
  • Current license (or license eligible), in the State of California.
  • Current BLS (CPR) through the American Heart Association.
