Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Orange Coast Memorial Medical Center. It is expected the employee demonstrate behavior consistent with our core values: Integrity, Accountability, Best Practices, Compassion and Synergy. The employee shall support Orange Coast Memorial Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities. The position requires seven days a week including holidays (major and minor) “on call” coverage on a rotating basis as determined by the Social Work Services Manager and the needs of the department. The Social Worker provides social work services to patients and families in all hospital areas specific to assignment. The Social Worker assists patients in addressing psycho-social issues they face as it relates to their acute or chronic illness and/or disability. The Social Worker is responsible for providing interventions to remove psycho-social barriers preventing safe and timely transition through the continuum of care. They skillfully advocate in empowering patients to maintain a level of control over their treatment and ensure self-determination. In addition, the Social Worker focuses on ensuring quality care while actively engaging in cost effective measures such as Performance Improvement Projects, LEAN activities and other hospital wide initiatives. Professional services include, but are not limited to biopsychosocial assessment, brief treatment, crisis intervention, patient education, discharge planning, custodial placements, and resource management.

Essential Functions and Responsibilities of the Job

  1. Conducts psycho-social assessments and interventions with patients and families.
  2. Addresses psycho-social issues and concerns through the continuum of care.
  3. Navigates systems and advocates to assist patient in accessing appropriate resources and services.
  4. Makes mandated protective services reports based on their psycho-social assessment of patient and family.
  5. Addresses barriers to a safe and timely discharge.
  6. Ability to be at work and be on time. It is required to be on the campus of the medical center during scheduled core business hours to accommodate issues which may arise out of a crisis and to support the medical team, peers and staff and their assignment to a specific area
  7. Ability to follow company policies, procedures and directives.
  8. Ability to interact in a positive and constructive manner.
  9. Ability to prioritize and multitask.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

Qualifications/Work Experience:

  • 1-2 years or more of direct social work experience in an acute healthcare setting preferred.
  • Competency is defined and measured via the Social Worker Job Specific Competency Assessment and Checklist. Each Social Worker, MSW is assessed at orientation and annually using these tools. 

Education/Licensure/Certification:

  • Master of Social Work (MSW) Degree from an accredited School of Social Work required.
  • Current American Heart Associate Basic Life Support Card
