Position Summary
In collaboration with Physician Leaders, Directors, Department Manager, and Supervisors. The Social Worker is responsible for supporting activities of MemorialCare Medical Foundations (MCMF) Care Management Program. Activities include but not limited to supporting patients/family regarding benefits, community resources, and referrals for social and/or behavioral health needs. This position will support patients while supporting the strategic goals of the MemorialCare Medical Foundation.
Essential Functions and Responsibilities of the Job
LANGUAGE SKILLS:Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.Ability to write routine reports and correspondence.Ability to speak effectively before groups of customer or employees of the organization.
REASONINGABILITY:Ability to apply commonsense understanding to carry out detailed but uninvolved written or oral instructions.Ability to deal with problems involving a few concrete variables in standardized situations.
OTHER SKILLS and ABILITIES: Proficient on Microsoft Word, Excel, Outlook and PowerPoint. Strong oral and written communication and research/problem solving skills. Attention to detail and accuracy.Ability to meet deadlines.Excellent interpersonal skills with a focus on internal/external customer satisfaction and teamwork.Proven dedication to quality, teamwork, customer service and professionalism.
Provide Social Worker support in the Outpatient settings (PCP office/Specialty office, etc.) and personnel serving MCMF Managed Care, IPA and ACO populations.
Adherent to regulatory compliance, contracted, delegated and non-delegated health plans supporting patient populations including but not limited to Medicare Advantage, Commercial, PPO, and ACO.
Provide evidence-based clinical care, patient-centered care, promoting efficiency in the delivery of value-based care and quality per scope and licensure.
Utilize Social Worker principles in the delivery of care. Be able to assist the patient in a sensitive and supportive manner, while acting as an advocate on behalf of the patient.

Follow company policies, procedures and directives. Maintain a strict degree of confidentiality as related to HIPPA and Compliance.
Interact in a positive and constructive manner as a member of the healthcare team.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements
Qualifications/Work Experience:
2+years of Social Worker Health Care related experience required
Working knowledge of NCQA (National Committee for Quality Assurance) SNP (Special Needs Plans); DMHC (Department of Managed Health Care), and other managed care regulatory requirements required.
- Working knowledge of various software programs including: MS Excel, MS Word, MS Project, PowerPoint, MS Outlook/Office, SharePoint and Adobe.
- Work experience in an Electronic Medical Record, required
- Bilingual in Spanish or another language preferred.
Education/Licensure/Certification:
Master’s degree in Social Work required
Licensed Clinical Social Worker (LCSW) preferred
