Position Summary
This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities.
The LCSW is responsible for providing clinical social work services to patients, families and groups in the Oncology Admin Services department. The LCSW is responsible for developing, implementing, and evaluating oncology support groups and education, coordinating the implementation of the Psychosocial Distress Screening tool, evaluation of tool across disciplines utilizing the tool and tracking and reporting to the Cancer Committee meetings. The position is responsible for the development, implementation, and evaluation of the Survivorship program at MemorialCare Saddleback Medical Center. This position is an integral part of the Oncology Admin. Services and the interdisciplinary oncology team.
Essential Functions and Responsibilities of the Job
1. Develops, implements, evaluates, and provides support groups for the oncology program. Reports data and support group outcomes.
2. Attends quarterly Cancer Committee Meetings to review current service line offerings enhancements and functions.
3. Implements Distress Screening tool utilization for cancer patients at each Transition of Care, through coordination with other SMC departments and/or services.
4. Coordinates an Oncology Survivorship program with a multidisciplinary team, that may include rehabilitation services, a registered dietitian, oncology nurse navigator, physician(s), palliative care team member and other services, utilizing the American College of Surgeon’s Commission on Cancer Survivorship Program Standard as the guide for program development.
5. Documents Psychosocial Assessment in accordance with expectations required of the LCSW and effectively prioritizes clinical interventions according to acuity level of psychosocial needs.
6. Collaborates with the healthcare team to provide education, resources and referrals to patient/families and caretakers.
7. Maintains knowledge and ethical practices concerning patient rights, confidentiality, self-determination.
8. Understands and applies appropriate, culturally competent interventions related to death and dying and to decision making issues at the end-of-life.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements
Qualifications/Work Experience:
2 years of Acute Hospital experience as an LCSW highly preferred.
Community Licensed Social Work experience desirable.
Education/Licensure/Certification:
Master’s degree from an accredited School of Social Work required.
Current California LCSW license required.
Knowledge of DSM-5 (Diagnostic and Statistical Manual of Mental Disorders) preferred
