Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities.

As a member of the Interdisciplinary Hospice and Palliative Team, the Bereavement Coordinator is responsible for the Hospice and palliative bereavement program, and oversees the counseling of hospice and palliative patients, caregivers and families and bereavement services provided.Counseling includes emotional, psychosocial and spiritual support and services provided before and after the death of the patient to assist with issues related to grief, loss and adjustment.

Essential Functions and Responsibilities of the Job

1.Develops and coordinates an organized bereavement program for families and residents of nursing facilities.

2. Oversees that the interdisciplinary team makes an assessment of survivor risk factors at the time of the patient’s admission and continually monitored and develops an appropriate plan for follow-up services needed.

3. Works with the Volunteer Coordinator to provide education and training for the volunteerswho provide services for the bereavement program.

4. Ensures that community bereavement support groups are provided weekly.

5. Supervises and ensures complete and current documentation of family assessments, plans and interventions

6. Provides bereavement education for staff and the community upon request.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

Qualifications/Work Experience:

  • Minimum of two years recent experience in a health care setting.
  • Advanced education/experience related to grief, loss, death, bereavement and counseling required.
  • Oncology or hospice experience preferred.

 

Education/Licensure/Certification:

  • Minister holding a Master’s Degree in Theology from an accredited school of Theology
  • One to two years post-graduate pastoral experience
  • Certified Chaplain or education in clinical pastoral counseling
  • Current California driver’s license with valid California car insurance and a good driving record.
