Position Summary

The Clinical Admissions Coordinator is a licensed professional who facilitates the process of patient admissions to ensure appropriate, timely, and smooth transition of patients into Long Beach Memorial Medical Center and Miller Childrens and Women's Hospitals. The Clinical Admissions Coordinator collaborates with the patient/family, members of the healthcare team, physicians, and payors to assure patients meet established criteria and are admitted to the appropriate level of care.

The RN, Coordinator – Clinical Admission (Transfer Center) is responsible for the clinical coordination and facilitation of patient transfer requests from external facilities. This role ensures timely review of clinical information, validates medical necessity and level of care, and collaborates with physicians, bed management, and case management to safely and efficiently accept patients.

Key Responsibilities

·Receive and clinically review transfer requests from referring facilities

·Assess patient acuity and determine appropriate level of care

·Validate medical necessity and payer requirements

·Collaborate with physicians, bed management, utilization management, and case management

·Facilitate smooth transfer workflows from acceptance to arrival

·Serve as clinical liaison between referring facilities and internal teams

·Reduce delays in transfer acceptance and bed placement

·Escalate complex or time‑sensitive cases appropriately

·Accurately document transfer coordination in the EMR

·Support surge planning and high‑census operations

·Participate in quality, throughput, and performance improvement initiatives

Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Qualifications:

·       3+ years direct patient care experience, with at least 2 years in an acute medical setting.

·       Healthcare professional candidates with 2+ years experience in utilization review, case management or discharge planning will also be considered.

·       One year CM (inpatient) or transfer center experience

·       Strong clinical assessment and critical‑thinking skills

·       Experience with patient flow, admissions, transfers, or care coordination

·       Knowledge of utilization management and level‑of‑care criteria

·       Proficient with electronic medical records (Epic preferred)

·       Excellent communication and multitasking abilities

·       Bilingual (Spanish preferred)

·       

Education/Licensure/Certification:

        Current California Registered Nurse License.

        Current American Heart Association (AHA) Basic Life Support certification (BCLS).

        Masters Degree in Social Work or Bachelors Degree in health related field preferred.
