Pay Range: $32.19/hr - $46.68/hr

Position Summary

The LVN works under the direction of the RN to coordinate the management of discharges requiring minimal intervention in the Discharge Lounge. Specifically, the LVN works within their designated scope of practice to perform nursing interventions to progress already developed discharge plans that enable patients who are assessed as medically fit to be discharged safely in a timely and safe manner. This role is integral to patients’ continuity of care specific to the discharge instructions for patients and their families as they transition home.

Essential Functions:

·         Prioritizes essential functions.

·         Demonstrates knowledge of the clinical information system and applies principles to patient documentation.

·         Contributes to an effective and safe working environment.

·         Serves as a role model and mentors new staff members.

·         Demonstrates knowledge and understanding of patient privacy rights. Maintains confidentiality of all medical, financial and other sensitive material in printed, electronic or verbal form, which may jeopardize the privacy of patients and or employees.

·         Demonstrates the ability to use good customer service skills by managing and acknowledging patient-family-significant other expectations.

·         Demonstrates tact, sensitivity, and discretion.

·         Demonstrates the ability to integrate knowledge of job specific competencies.

·         Adheres to infection control, confidentiality and safety guidelines and policies, observes all safety and health regulations, and ensures compliance with regulatory agency standards and State and Federal regulations.

·         Reports any patient problems encountered to the RN, physician and/or manager as appropriate.

·         Contributes to professional development of peers and colleagues by sharing knowledge and skills.

·         Maintains positive working relationships with patients/families and interdisciplinary team members. Demonstrates consistent professional attitude and behaviors. Practices the principles of effective communication. 

·         Responsible for maintaining professional competence and keeping informed of changes.

·         Ability to be at work and be on time.

·         Ability to follow company policies, procedures and directives.

·         Ability to interact in a positive and constructive manner.

·         Ability to prioritize and multitask.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years 20 experience and qualifications. Minimum Requirements

  • 2 year experience working as an LVN in an in-patient setting required.
  • Experience of multidisciplinary working/discharge planning in acute services preferred
  • Highschool diploma or equivalent required
  • Current licensure to practice as a Licensed Vocational Nurse in California.
  • IV certification required
  • Basic Life Support (BLS) required.
